If we do not receive an executed resale/exemption certificate or documentation from your company, we will be required by the state(s) to charge and collect sales tax from you and remit such tax to the state(s).

Please complete the form below and email the applicable Resale or Exempt on Form(s) for all the states where you maintain sales tax exempt status to the email address below. We have also provided our fax number and mailing address if you prefer to send us hard copies.
